We are unable to process claims or bill insurance for any medication. However, certain companies or plans may reimburse for compounded prescriptions. Please contact your insurance provider and follow their instructions for reimbursement. If they require information from the pharmacy, we are happy to provide a Universal Claim Form for you to submit. Please indicate your preference below:
